• Basic company information, including physical mailing address and invoice/payment email addresses
• Owner information
• Federal ID number (FEIN)
• Tax-exempt information (if applicable)
• W9 form
• CLIA certificate (if applicable)
• State business license (if applicable)
• Prescription drug and/or prescription device licensing (if applicable)
